Real social influencers are not found on YouTube® channels or Facebook® feeds. Instead, they are found in communities struggling with food insecurity, housing instability and homelessness, high rates of crime, social isolation, and lack of the most basic healthcare services. Since 2016, HealthEC® and the Prince George’s Healthcare Alliance© in Maryland have worked together to break down barriers to healthcare and build stronger communities by identifying social factors and behaviors that impact access and harmonizing interventions to improve the quality of care coordination for high-need, high-utilizer populations. Their combined vision and strategic partnership have helped to improve healthcare outcomes by meeting patient needs far beyond medication and primary care. Join Barbara Wiggins-Banks of the Prince George’s Healthcare Alliance© and Sita Kapoor of HealthEC® to learn more about how these two organizations have pioneered the way forward to effectively address SDoH needs by developing the non-profit’s ability to identify beneficiaries most in need, integrate and execute effective interventions and care transitions, and evaluate patient outcomes and ROI to pinpoint opportunities to reduce costs and optimize resources.
