TP-ACO experts, Jay Simmons, MS-ISM, Vice President, Physician Services, Lauren Stuteville, RN, BSN, Transition of Care Coordinator, and HDAI’s Julie Woods, Manager of Customer Success, will share how an interdisciplinary team collaborated to successfully use HDAI’s HCC Recapture tool to address and optimize Hierarchical Condition Categories (HCCs) capture within their ACO population.​

HCCs aren’t new, but with the recent shift towards value-based care, health systems and ACOs are pushing towards more complete capture rates. Accurate coding improves the capture of the true acuity of your patient panel and ensures proper reimbursement.

Learning Objectives: ​

  • Understand the purpose and value in accurate HCC coding in value-based care, specifically Medicare ACO arrangements, and how HCCs impact Risk Adjustment and performance year benchmarks. ​
  • Learn how HDAI’s HCC optimization helps to better understand patient patterns related to condition diagnoses, what diagnoses are most likely to reoccur, and how diagnosis weight play into calculations with minimal lift for clinicians.​
  • Gain insight into strategic operations of an HCC initiative – what works, what doesn’t, and how to iterate based on data-driven results and provider input.
