Digital health platforms are a natural fit for Accountable Care Organizations given the focus on remote monitoring and behavioral change enablement that drive higher quality outcomes at lower costs of care. Yet many ACOs have not begun to leverage digital tools within their suite of offerings while payers, employers and health systems have charged ahead with innovative adoption to drive population health at scale. ACOs have much to gain by leveraging digital health within their organization and those that adopt digital health solutions sooner will reap the benefits of higher quality care and increased shared savings. Additionally, an evolving focus on equity continues to pressure ACOs into addressing social determinants of health and to identify ways to remove barriers to care for medically underserved communities. This webinar will feature a leading ACO expert who will lay out the challenges facing ACOs at delivering population health at scale and improving health equity metrics. Digital health panelists will then round out the discussion, providing insight on how digital platforms can be leveraged to overcome these challenges and help ACOs succeed in shared savings models.
