When a patient is admitted to the hospital, much attention is paid to the quality of care she receives throughout the care encounter. Less is made of what happens after the patient leaves the hospital and the discharge planning that goes into accessing post-acute care.

During discharge planning, medical providers review self-management routines with patients and their family caregivers, outline medication regimens, and ensure all parties have received the education necessary to achieve optimal outcomes after the patient leaves the hospital.
