To reduce asthma emergencies, Medical Home Network ACO is replacing Medicaid patients’ gas stoves with electric stoves
Medical Home Network (MHN) and the MHN ACO are launching an asthma prevention program that will replace gas stoves with electric stoves for MHN ACO Medicaid patients.
Chicago-based MHN is focused on transforming care in the safety net through value-based care arrangements. It is helping FQHCs build whole-person care coordination capabilities at the practice level — powered by AI-driven, real-time analytics and information that gives a full picture of a patient’s health.
MHN ACO, whose members include 14 federally qualified health centers, three hospital systems, and their physician practices, said the stove initiative will address a known environmental trigger in a population where asthma drives 1,800 annual ED visits and $3.5 million in direct costs.
